We provide an in-home assessment consultation to pinpoint which personal care services are needed if this has not already been determined, Home Health Aides, or professionally trained health care providers, are matched based on the required level of skill, personality, and availability. Once chosen, the care provider is familiarized with the client and given details of special needs (likes and dislikes) and requirements. After a provider is placed we follow up with monitoring and weekly review.
In-Home Health Care Support and Facility Care Support Guidelines
We provide non-medical in-home health care support services for senior citizens and the disabled. We do not replace a visiting nurse or a Physician. Our team of experienced and dedicated certified nursing assistants and home health aides perform healthcare support obligations.
Description Home Health Support Services
Once released from a hospital or rehabilitation facility, written aftercare instructions are given. The instructions are to be repeated daily or on a regular basis at home by the individual or a care provider. We follow the aftercare instructions given by the hospital or rehabilitation facility. Care, as prescribed, will aide to maintain or improve the health, well-being, or comfort of the individual. A few examples are post-surgery wound care - Alzheimer's care - quadriplegic care - dietetic meal preparation - medication reminders - charting vital signs. Our care services are designed to assist one to continue care from a home environment.
Weekly Rates and Monthly Rates Weekly are available upon request.
Price Quote Once the service to be provided is determined and agreed upon, we will present a detailed price quote.
The contract we will present a contract for signing before care services are scheduled to begin. All contracts are complete with a detailed, list of care services to be performed, price, and length of time. The length of a contract may be extended at any time.
Contract Price Will depend on the service needed. Most are limited to 5 days per week. (Contact us for information on 24-hour rates). The week can be adjusted through the month to fit the schedule of the client or loved one. Example: (Tuesday through Saturday) Monthly contract rates are more cost-efficient than hourly or weekly rates.
Contract Termination A contract may be terminated, by either party, with good cause, or by mutual agreement with a thirty-day notice. A 10% cancellation fee will apply. Good Cause Example: Service is no longer require, moving out of the service area, etc..
